WHAT IS THE PRE-SIOP INTAKE FORM? WHAT DO I NEED TO COMPLETE IT?
The Pre-SIOP Intake Form (available on our website) is a required digital form for each student to attend the conference. This should be completed after the student completes the registration form (step 1) for the conference.
This form requires a CALIFORNIA SSID, which is usually viewable on the student's grades portal (such as Q, StudentVue, PowerSchool, etc). If not viewable, it may also be on the student's recently printed report card or transcript. The student may also request this information from a counselor or potentially from a teacher.
The pre-event questionnaire is a requirement from the UC Office of the President, who provides the funding for these programs. The information in those forms is not received by El Congreso, nor will it be shared with anybody outside the university. It is for statistical purposes that evaluate the efficiency of outreach programs in the present and future (i.e. how many students eventually enroll in community college or public state university).
The form will also ask for other demographic information, such as race, ethnicity, school code, etc. Again, if you are not sure about what to write for these responses, please talk to your teacher or counselor.
Undocumented students do not need to worry about this form. The University of California is a sanctuary for undocumented students, has protected the community against unfair policies, and will not ask for information regarding status. The UC welcomes them as prospective students and supports them with financial aid, offers services such as the Undocumented Student Services, and offers immigration advice to enrolled undocumented students and their direct family free of charge.
